STEP 5: Spouse/partner call. If the spouse or partner was not previously involved on calls, this is required to proceed. Complete your pro forma template.

Aging & Health reports that studies have shown the deliver of home or community-based long term care services is a cost-effective alternative to nursing homes.

Medicare is the largest single payer of home care services. About 32% of annual home care expenditures are paid for by Medicare, a little over 18% are paid for out-of-pocket or by private insurance and approximately 13% are covered by Medicaid (National Association for Home Care).
